Before you use it
- Every adult who will live in the home should complete their own application and sign the screening consent.
- Ask only what you need to apply written criteria, and never ask about traits that fair housing law protects.
- Many states and cities cap application fees, limit criminal history questions, or protect income from vouchers.
- If a consumer report affects your decision, federal law requires an adverse action notice.
How to fill it in
- Underlined spaces are blanks to fill in. The small label says what goes there.
- Shaded boxes mark rules that change by state. Look up the rule where the property is located.
- Delete clauses that do not apply, and add any disclosures your state or city requires.

8. Screening criteria
Our written criteria are attached or available on request. They cover income, rental history, credit, and any other checks we run, and they are the same for every applicant.
9. Authorization and consent
I confirm the information in this application is true and complete. I authorize the landlord and its screening provider to verify it, contact my landlords, employers and references, and obtain consumer reports, which may include credit, eviction and criminal history where the law allows. If the decision is based in whole or part on a consumer report, I will receive an adverse action notice with the name of the reporting company and my right to a free copy and to dispute it.
I understand false or missing information may be a reason to deny this application or end a lease. Approval is not final until a lease is signed by all parties and the required deposit and first month's rent are paid.
